If you are referring a participant, patient, or client, this page sets out everything you need: turnaround times, what to send, current capacity, and how we work with you.

Current capacity — open for new referrals

FCAs & AT reports: 4-week turnaround.

First telehealth appointment: usually within 7 days.

In-person across the Northern Rivers: usually within 10 days.

How to refer.

Two ways. Whichever is easier for you.

Option 01

Send the participant's name, contact, plan management type, and a brief description of what you need to info@functionalpathwaysot.com.au.

Call us on 0413 260 347. If we don't pick up, leave a message and we will call you back.

What to send with a referral.

The minimum we need to get started:

  • Participant's name and contact details (with consent to be contacted by us).
  • NDIS plan dates and management type (self / plan / agency).
  • The reason for the referral — the question you want our work to answer.
  • The funding category you have in mind (most often Capacity Building - Improved Daily Living).
  • Any urgency context — plan review date, hospital discharge timeline, equipment quote deadline.

Helpful but not required: any prior reports, medical context, or specific assessment requirements (eg. a particular standardised tool).

What you can expect from us.

  • A reply to every referral, confirming whether we can take it and an honest estimate of timeline.
  • Service agreement to the participant within 7 days of the screening call.
  • Reports returned within four weeks for most assessments — we will tell you upfront if a complex case will take longer.
  • Notes and progress updates on request.
  • Direct invoicing of plan managers using NDIS-compliant invoice templates.

What we cover.

Functional Capacity Assessments

Plan reviews, access requests, change of circumstances. Clear summary plus clinical detail.

Assistive Technology

Equipment prescription — wheelchairs, adjustable beds, transfer aids, hoists, mobility aids. Levels 1 and 2 by default. Level 3 by arrangement.

Home Modifications

Grab rails, ramps, level-access showers, doorway widening, lever taps, accessible kitchens. In-home assessment and report.

Cognitive Therapy

Practical strategies after stroke, ABI, MS diagnosis, post-concussion, or in early cognitive change.

Yoga Therapy

OT-led, goal-tied, billed under the standard OT line item.

NDIS Access Requests

Functional impact reports for adults applying to the NDIS for the first time.

Telehealth

For all of the above where it is appropriate. Australia-wide.

Working details.

  • Plan management: Self-managed and plan-managed participants. Not currently agency-managed (unregistered NDIS provider).
  • Geographic reach: In person across six Northern Rivers LGAs (Byron, Clarence Valley, Kyogle, Lismore, Richmond Valley, Tweed), up to Tweed Heads on the Queensland border. Telehealth Australia-wide.
  • Billing: NDIS Pricing Arrangements rates. Travel within NDIS limits.
  • Worker Screening: Current NDIS Worker Screening Check (NSW).
